Excellent idea. The way HRQoL is currently set up is confusing, in part due to the reasons you mentioned. The current system leaves room for manipulation of interpretation that appears favorable to the drugs when, in reality, patients are miserable.

I'm curious if HRQoL questionnaires have benefited from the kind of rigor that psychometricians subject their neurocognitive, behavioral and clinical questionnaires to. Usually the latter have been studied with large sample sizes, tested in large epidemiological surveys in a wide variety of populations, subjected to pretty elaborate reliability and validity tests, etc..
